29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired. On or about 4/23/2015 at the left side and left front of the building; employees were exposed to fall and scaffold collapse hazards where: a. the left side, 2nd frame, lower, inboard is bent, b. the left side, 1st frame, lower, inboard is bent, c. the left front, last frame, upper, inboard is bent, d. the left front, last frame, outboard walk board is badly cracked, e. the left side , 1st frame, 2nd outboard, board is visibly cracked span wise, f. the left side, 1st frame & 2nd frame, outboard walk board is visibly cracked span wise, g. the left side, 2nd frame, inboard walk board is visibly cracked span wise, h. the left side, 3rd frame, outboard, deck board in visibly missing a portion of the board.
