29 CFR 1910.179(n)(3)(xi): The warning signal was not sounded whenever the bridge was started or when the load or hook approached near or over personnel: On or about at 1/17/15 located at 1527 Augusta Ave., Augusta, GA: Employee exposed to a struck-by hazard from the overhead bridge crane not equipped with a warning signal device. The employer failed to have in place a warning signal sound to warn employees working in the area of the overhead crane being operated.
