29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a. On or about January 10, 2013 - at the above addressed jobsite, employees were lifting large farming equipment using a fabricated lifting device which is attached to the forks of a SKYTRAK 8042 Rough Terrain Forklift. The truck is not marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered, thereby exposing the employees to struck by hazards due to forklift tip over.
