29 CFR 1910.29(b)(2): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: WORKSHOP: The employer had not ensured that midrails were installed on all open sides of the wooden crate/basket (platform) attached to the forks of the Toyota No. 6 powered industrial truck (PIT) to protect the employee from falling approximately twenty (20) feet to the lower level while changing ceiling light bulbs and removing cob webs from the platform.
