29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: On or about March 19, 2013, roofers installing decking on a roof were accessing the roof and working from site-built scaffolding greater than ten feet high at the back without it being fully planked exposing employees to the hazard of falling.
