29 CFR 1910.244(a)(2)(i): Where the employer did not ensure a firm foundation was provided and where there was a possibility of slippage of the cap, a block was not placed in between the cap and the load: (a) On or about November 18, 2019 for an employee engaged in equipment maintenance and repair activiites who was fatally injured when a CAT 930 Pay Loader slipped off a bottle jack during wheel removal at 1416 Eleventh Avenue SE in Watford City, North Dakota.
