29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about November 21, 2017, employees performing cleaning operations on the re-feed conveyor were exposed to caught-in hazards and the employer failed to provide lockout/tagout procedures that clearly and specifically outlined the procedures to shut down the equipment.
