29 CFR 1910.36(d)(1): Employees were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors: On or about November 3, 2017, employees in the Breeding Barn (Barn 5) were exposed to smoke inhalation and burn hazards when the exit doors could not be opened: a) North Exit Door - secured by a wire attached between the handle and door jamb; and b) South Exit Door - missing the handle and deadbolt knob needed to operate the closure mechanisms.
