29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provided timely warnings before the minimum approach distance required by paragraph (d)(1) of this section was reached: (a) On or about 04/26/2017 - Fulton Springs Substation, 1452 Leora Road, Fultondale, AL, the digger derrick truck operator was not warned by the spotter employee about reaching the Minimum Approach Distance before the derrick boom made contact with the 7,200-volt energized power distribution line.
