Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught between, struck-by and fire hazards while operating a scissor lift in an area and manner that did not maintain clearance from overhead obstructions. On or about June 14, 2021, the employer did not ensure that adequate clearance was maintained between a scissor lift and overhead obstructions while operating a scissor lift between cooking kettles (reactors) R601 and R604. While operating the scissor lift, it struck a pressure tap on a hot oil return pipe for cooking kettle R604, resulting in an oil leak and subsequent fire. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
