29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: The employer is failing to protect employees from fall hazards when employees are utilizing a cat walk more than 4 feet above ground. This was most recently documented on or about January 29, 2021 at 3200 Enterprise Avenue, York, Nebraska when employees were observed walking across a catwalk in the Zone 2 of the facility that was missing a portion of its top railing.
