29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: a) Vestas Blades America, located at 1500 East Crown Prince Blvd. Brighton, CO 80603: On and preceding February 17, 2019, the employer did not ensure that H-frame brackets were stable and safely arranged while being transported using powered industrial trucks. Each H-frame measured approximately 9' x 9' and weighed approximately 1500 pounds. Employees used powered industrial trucks to move stacks of up to 5 H-frames at a time by sliding the forks through the middle of H-frames. When the H-frames were loaded unto the forks, employees working on the ground were regularly required to remove ratchet straps when H-frame bundles were elevated by the forklifts. The H-frames were periodically misaligned, causing the load to become unstable and sway while being handled and transported. This condition exposed employees to struck-by hazards.
