29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes , more than six feet (1.8 m) above lower levels by personal fall arrest systems, covers, or guardrail systems erected around such holes: A) 220-250 Westfield Ave Roselle Park, NJ 07204 Employees were exposed to a fall hazard of approximately 20ft, when a floor opening with an extension ladder used to access interior courtyard was unprotected.
