29 CFR 1910.110(e)(4)(iii): LP gas container(s) installed on internal combustion engine vehicles were not securely mounted to prevent jarring loose, slipping or rotating: a) On March 29, 2017, the employer failed to ensure that the Toyota (18), serial #60895, powered industrial truck had a functional bracket to secure the liquefied petroleum gas (LPG) tank to prevent the tank from jarring loose and slipping. Try Our Pallets, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.110(e)(4)(iii), which was contained in OSHA inspection number 974570, citation number 1, item number 2 and was affirmed as a final order on June 17, 2014, with respect to a workplace located at 37 South 9th Avenue, Maywood, Illinois 60153. In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
