Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hoisting/ rigging hazard, struck-by hazard when lifting a 2000 lb steel plate with a manual chain hoist: Location: 519 8th Ave, New York, N.Y. On or about 2/27/2017 The employer was constructing a steel tank 18 ft long by 8 ft wide by 16 ft tall; when attempting to lift a 2,000 lb steel plate with a manual chain hoist the hoist dragged the steel plate sideways striking an employee. a) The employee operating the manual chain hoist was not standing in a safe location clear of the load; the hoist chain was not rigged long enough to allow the operator to stand off to the side of the 8 ft wide steel plate being lifted. b) The hoisting chain was connected to the steel plate by tieing the chain in a knot; rigging shackles were not used. c) The manual chain hoist was not centered over the load; was 4ft off center causing the hoist to drag the plate sideways; nor was a tag line used to control the load being lifted. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS: Comply with industry standard ANSI/ASME B30.16 Overhead Hoists; Installation, Use and Inspection which states: --- par 16-1.3.3 which states: Hoists shall be installed in locations that will allow movement of the operator to stay clear of the load. --- par 16-2.1.2 c) (6) which states: Hook latches, if used, shall be inspected for proper operation. --- par 16-3.1.2 b) which states: The load shall be attached to the load hook by a suitable means. --- par 16-3.1.2 g) which states: Hoists shall not be operated unless the hoist is centered over the load& Should it be necessary to pick a load which is not centered under the hoist unit, precautions shall be taken to control the swing of the load when it is picked clear of its support. NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
