29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) ADA Dash Subline - A06-1/A06-2; On or about June 18, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #2 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip. b) ADA Dash Main Line - A900; On or about June 18, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #9 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip. c) JFA Quarter Line L/H - A06/A101/A05; On or about June 17, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #28 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip.
