29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) ADA Dash Main Line - A900; On or about June 17, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #30 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip. b) JFA Quarter Line LH - A01/A02/A03/A05; On or about June 17, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #30 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter a robotic cell where there were energized robots and other jigs and fixtures to replace a welding tip. c) ADA Dash Main Line - A900; On or about June 18, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #34 was exposed to caught-in, struck-by, and crush hazards in that the employee was allowed to enter a robotic cell where there were energized robots and other jigs and fixtures to clear a sensor fault. d) ADA Dash Main Line - A700; On or about June 18, 2016 and at times prior thereto, where procedures had not been developed, documented, and or utilized for the control of potential hazardous energy, Employee #34 was exposed to caught-in, struck-by, and crush hazards in that the employee was allowed to enter a robotic cell where there were energized robots and other jigs and fixtures to clear a sensor fault.
