OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by/crush hazards and fall hazards. a) Fuel tank farm site - On or about 12/1/15 and at times prior thereto employees assigned to the removal and relocation of tanks conducted welding, cutting and dismantling operations on top of tanks unstable ladders and catwalks. The employer did not perform a Job Safety Analysis (JAS) to determine the stability of the different structures. A safe sequence of actions was not followed and coordinated with employees and contractors in advance in order to prevent the accidental collapse of any part of the installation. Among other methods, one feasible and acceptable method to correct the hazard is to follow the recommendations described below: 1) Prior to starting the work, conduct a Job Safety Analysis (JSA). Assess the hazards inherent to the operations for the removal and relocation of the fuel tanks, and develop the corresponding safety strategies and protective measures. 2) Review and coordinate the sequence of actions and protective measures with employees and contractors in advance to prevent exposer to the fall hazards and struck-y/crush hazards.
