Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards of being struck-by, crushed, and/or caught in moving parts under a concrete pump truck: a) 906 Simpson Street, Bronx, NY 10459: On or about January 6, 2015, a driver and an operator of a concrete pump truck were pumping concrete to a construction site when they were exposed to hazards of being struck-by, crushed, and/or caught in the rotating main drive shaft of the truck as the driver went under the truck in an attempt to trouble-shoot problems with the pumping of concrete. Among other methods, feasible and acceptable abatement methods to correct these hazards include: 1. Develop, document and utilize a lockout/tagout program in accordance with ANSI/ASSE A10.44-2006 Control of Energy Sources (Lockout/Tagout) for Construction and Demolition Operations.
