Abstract: Employee #1, a foreman, was on the roof of a building under construction at the Cleveland Browns training facility in Berea, Ohio. He had climbed to the top of the roof to get an extension tool for his son, who was on a scaffold at the roof level. When Employee #1 descended and stepped onto the bottom tier he slipped on the painted surface of the roof's sheet metal panel and slid off the edge. Employee #1 fell approximately 40 feet and died. Rubber mats were provided on three of the roof panels to aid workers who were climbing up and down the tiered roof. Employee #1 was wearing a safety belt, but was not tied off.
