Abstract: At approximately 10:56 a.m. on November 9, 2002, Employee #1 and several coworkers were working at an automobile repair shop. Employee #1 was located under a van (Make: Nissan; Year: 1997), on a creeper, replacing motor mounts. The front end of the van was elevated with jack stands, while the rear was resting on its wheels. Employee #1's coworkers were troubleshooting a vehicle (Make: Volkswagen; Model: Beetle) just outside the covered garage area where he was working. Further, a tool cart was placed between the rear of the van and the Volkswagen, Beetle. A coworker started the Beetle by reaching in and turning the ignition key. For some reason the manual transmission was in first-gear, causing the vehicle to lurch forward and hit the tool cart. The tool cart rolled into the van and knocked it off the jack stands. Employee #1 was struck by the van's transmission case in his left temple. Employee #1's coworkers manually lifted the van and pulled him, while still on the creeper, from under the car. Emergency services were contacted and Employee #1 was transported to the USC Medical center where he was hospitalized and treated for his injuries. Employee #1 died two days later.
