Abstract: Employee #1, an apprentice carpenter, was removing falsework from underneath a freeway connector under construction. The falsework consisted of wooden forms resting on loose steel beams which were resting on vertical steel structures called bents. During this operation, while the bents are being lowered so that the beams can be removed, the wire rope guy lines that act as braces should be kept tight. On this day, the carpenters removed all the wire rope guys on one side of the beam and loosened all the guys on the other side of the beam. The whole structure collapsed domino style, and a 36 ft beam crushed and killed Employee #1. A qualified supervisor was not at the job site.
