At 9:00 a.m. on August 7, 2018, Employee #1, employed by a construction company, was working at a multiemployer construction project, a one-story residential st ructure. He climbed a Werner Model D1116-2 ladder. The ladder slipped and kick ed out, and the employee fell, a fall height of 13 feet. The employee was trans ported to the hospital, admitted, and treated for fractures to both heels.
