29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Latitude 32.9863253, Longitude -90.5892611 on the North side of Louise, MS - On or about August 17, 2013, an employee climbed a cellular tower to a height of about 125 feet wearing a body harness but did not have a lanyard attached to the upper back D-Ring or the safety slide guard for the front D-Ring.
