29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: a. On or about February 13, 2013, the employer did not ensure all authorized employees affixed lockout devices to the energy control points. Two employees were working on the main shredder with only one lock applied to the lockout point on the main shredder. b. On or about June 19, 2013, the employer did not ensure all authorized employees affix lockout devices to energy control points. Multiple employees are required to work on the main shredder and small grinders according to company policy and only one lock is applied to energy control points due to only one lock will fit the lockout point.
