Text For Citation: 02 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the A-Frame and/or components of the A-Frame system on the Kenworth C-500 truck: a)D & D Water Service & 3 miles N.E. of Strasburg Road and 88th Ave., Strasburg, CO: On or before May 13, 2003, the employer did not ensure that the damaged dogbone on the A-Frame of the Kenworth C-500 truck was maintained in serviceable condition. The dogbone and other components of this hoisting system were not adequately inspected and repaired. As a result of not inspecting and maintaining the hoisting system and the dogbone structural member, employees were exposed to the hazard of being struck by the A-Frame and/or components of the A-Frame system. The failure of this system did in fact result in the death of an employee who was working for another contractor. Among other methods, one feasible and acceptable method to correct the hazard is to conform with the requirements of American Petroleum Institute Recommended Practice 54 1999, Section 9, Sub-Section 9.2, Paragraph 9.2.4, American Petroleum Institute Recommended Practice 54 1999, Section 9, Sub-Section 9.2, Paragraph 9.2.8, American Petroleum Institute Recommended Practice 4G 1998, Section 5, Paragraph 1a, American Petroleum Institute Recommended Practice 4G 1998, Section 3, Paragraph 3.2, International Association of Drilling Contractors Accident Prevention Reference Guide 1998, Section 17, Paragraph 17.1F, and International Association of Drilling Contractors Accident Prevention Reference Guide 1998, Section 17, Paragraph 17.1G. These industry standards outline the requirements of inspecting, repairing and maintaining the structure, and the rigging, of the truck mounted A-Frame (mast) hoisting system to include but not limited to the dogbone structural member and the clevis mounted on the "headache rack".
