29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: (a) Authorized employees set up and performed maintenance on the two vertical injection molding machines without first affixing a lockout device to the energy isolating devices such as the electrical disconnect and the pneumatic ball valves and blocking the gravitional energy of the ram.
