Text For Citation: 03 Item/Group: 011 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to death or crushing hazards from falling loads caused by bolt or shackle failure: a) Sheet and Tin Division 1) On numerous occasions case hardened bolts have been used to replace shackle screw pins. The bolts will shear rather than bend as would the original pin. This shearing action would eliminate all forewarning of failure and a possible falling load, observed June 20, 1989. b) Iron and Transportation Division 1) Open Hearth Mobile Shop - Clevis 7/8" regular bolt replaced by a 3/4" hardened bolt and nut, observed August 31, 1989. Among other methods, one feasible and acceptable abatement method is to use only those pins designed for use with shackles.
