Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the two hand controls were not arranged by design or construction or separation or both to require concurrent use of both hands: a) Utilities, Shop, and Services 1) Central Maintenance Shop - Fabrication Department - The two (mushroom type) palm buttons used to initiate stroke of the Steelweld (large) shear, serial number M4850, were only separated 13 and one-half inches.
