29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about July 27, 2023, at the Tire Room- Area B- Channels 1- 8, employees were performing servicing and maintenance activities on such as but not limited to a gripper arm and unloader of a DRB #8201 tire machine. Hardware was not provided to secure and/ or block the horizontal movement of the DRB tire machine components.
