29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 24, 2023, in the Main Shop and Tank Shop, employees performed various servicing such as set up, blade changes, and cleaning on equipment including, but not limited to press brakes, band saws, plasma tables, slip rollers, and punches used for the fabrication of tanks on the following instances: a) AccurPress Hydraulic Press Brake, Main shop, east wall. b) Hyd-Mech Bandsaw, Tank shop, south wall. c) BOSS Plasma Table, Main shop, east wall. d) MPR-E Series Slip Roller. Main shop, east wall. e) GEKA Punch, Tank shop, south wall.
