29 CFR 1910.269(m)(3)(vii): The employer did not ensure the installation of protective grounds as required by paragraph (n) of this section: a) On or about July 13, 2023, and at times prior, LND Technical Services, LLC, did not require the use of temporary personal protective grounds on the Medium Voltage (MV) side of Transformer T201 during reconnection of cables to terminal windings after high voltage-inducing tests were completed on the MV side. This exposed employees to electrical hazards. b) On or about July 13, 2023, and at times prior, LND Technical Services, LLC, did not require the use of high voltage (HV) side, built-in grounding disconnect switches on Transformer T201 during High Voltage (HV) side reconnections of bushings to conductors after high voltage-inducing tests were completed on the HV side of Transformer T201. This exposed employees to electrical hazards.
