29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards. On or about July 13, 2023, at 1096 N Venetian Drive, Miami Beach, Florida, 33139, on the north side of the house, employees working from elevations on the scaffold system were not adequately trained to recognize hazards and methods to control those hazards, exposing employees to a fall, struck by and/or crushed by hazard.
