29 CFR 1910.120(q)(3)(i): All emergency responders and their communications were not coordinated and controlled through the individual in charge of the incident command system, assisted by the senior official present for each employer: On or about May 2, 2023, the employer failed to ensure that all emergency responders and their communications were coordinated and controlled through the individual in charge of the incident command system (ICS) when employees were required to respond to two releases of hazardous substances with the potential for health hazards, including, but not limited to, N-Methylpyrrolidone (NMP) and the employer did not establish an ICS.
