The lack of training resulted in employees being exposed to fire, explosion and hydrogen sulfide hazards. c) BP Products North America, Inc. - Oregon, Ohio: On or about September 20, 2022, the employer failed to ensure that inside and outside operators were trained on operating limits of the TIU Fuel Gas Mix Drum, related to liquid level, including the consequences of deviation and steps to avoid and correct liquid level outside of acceptable limits. The lack of training resulted in employees being exposed to fire, explosion and hydrogen sulfide hazards.
