On or about July 8, 2022, an OSHA Form 301 or equivalent for the following work-related injuries or illnesses was not filled out for the calendar year 2022. (a) Bleach Plant Operator suffered illness related to chlorine dioxide exposure that occurred on or about June 13, 2022, resulting in the employee's death on June 27, 2022.
