29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: a) 11600 W 2nd Pl, Lakewood CO 80228, St. Anthony Hospital CATH Lab 3: On or about March 24, 2022, employees were exposed to shock hazards from trying to dismantle a 277-volt 4 square electrical junction box while circuit breaker #21 was still energized with 277 volts and was not tagged out.
