29 CFR 1910.212(a)(2):General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a. On or before February 8, 2022, the employer failed to ensure employees were protected from the rotating shaft the Lid Spinner Machine #2. The existing guard that was previously installed created an incoming nip-point hazard, thereby exposing employees to a caught-in hazard. b. On or before February 8, 2022, the employer failed to ensure employees were protected by guards that were secured by a compliant fastener on Printing Machine 8. The guard on the side of the machine was held closed with Velcro, thereby exposing employees to a caught-in hazard. c. On or before February 8, 2022, the employer failed to ensure employees were protected by guards that were secured by a compliant fastener on Printing Machine 9. The guard on the side of the machine was held closed with Velcro, thereby exposing employees to a caught-in hazard. d. On or before February 8, 2022, the employer failed to ensure employees were protected by guards that were secured by a compliant fastener on the Lid Printer 7. The guards on the side of the machine were held closed with Velcro, thereby exposing employees to a caught-in hazard. e. On or before February 8, 2022, the employer failed to ensure employees were protected by guards that were secured by a compliant fastener on Lid Printer 7. The guard on the front of the machine was held closed with a sliding latch, thereby exposing employees to a caught-in hazard.
