29 CFR 1910.160(b)(13): Automatic detection equipment of fixed extinguishing systems was not approved, installed and maintained in accordance with the requirements in 29 CFR 1910.164: a) Boulder Scientific Company, LLC., 598 Third Street, Mead, CO 80542: On or about October 22, 2021, the fire suppression system in the Mars Lab did not activate automatically. Employees who used soda ash from an container outside of the lab to extinguish the chemical fire were not protected from fire hazards and/or burns.
