29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by by means of covers, guard rail systems, travel restraint systems; or Personal fall arrest systems: a) On or about 10/14/2021, and at times prior, 123 Harbour Drive, Scottsboro, AL 35769, employees who were performing roof repairs were exposed to fall hazards up to 30 feet from the metal roof to the ground below because the employer did not protect each employee from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by means of covers, guard rail systems, travel restraint systems; or Personal fall arrest systems.
