29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee was protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level: (a) Lexmark International Inc. at 6555 Monarch Road in Longmont, CO: On and before July 2, 2021 the employer did not protect workers from stepping into holes in the second and third floors of the pilot plant where chutes had been removed and covers had not been affixed. Employees were exposed to trip and fall hazards.
