29 CFR 1910.178(q)(1):Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: (a) On or about 5/27/2021, 1901 13th Avenue East, Tuscaloosa, AL 35404, in the stockroom area - employees using Work Assist Vehicle (WAVE) platform were exposed to fall hazards up to 17 feet to the concrete floor because the vehicle was operated with a defective safety gate.
