29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose; and each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: On or about April 28, 2021, employees were exposed to electric shock hazards in that: a. In the packaging room, a junction box did not have a cover. b. In the THMR Kwikcutter 449 mixer loading station, at least two outlet boxes did not have covers.
