29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: On or about April 28, 2021 employees were exposed to electrical shock hazards in that: a. In the mill shop, the electrical cabinet was missing at least two knockouts and the cabinet doors were not effectively closed. b. In the mill shop, the disconnect and junction boxes for the Van Dale mixer were missing knockouts.
