29 CFR 1904.40(a): The employer failed to provide copies of the injury and illness records to an authorized representative within four hours of the request: On or about November 11, 2020, an explosion occurred during the manufacturing of a water-activated product used to generate chlorine dioxide resulting into a fatality and work-related injured. The employer did not provided the OSHA 300 (Work-related Injury and Illness Log), the OSHA 300A (Summary of Work -related Injury illness Form), and the OSHA 301 (Injury and Illness Incident Reports or equivalent forms) for the calendar year 2020 within four hours of the request. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
