29 CFR 1910.1200(h)(3)(ii): Employees were not trained on the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area: On or about November 11, 2020, an explosion occurred during the manufacturing of PureVista a water-activated product used to generate chlorine dioxide and the employer did not ensure that employees and management officials were provided effective training on hazards to which they were exposed. Effective training lacked one or more of the following elements: training on elements of combustible dust fire, deflagration, explosion hazards, ignition sources, hazardous outcomes, and potential injuries. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
