29 CFR 1926.652(c)(2)(i): Design of support systems, shield systems, or other protective systems that are drawn from manufacturer's tabulated data were not in accordance with all specifications, recommendations, and limitations issued or made by the manufacturer: a) Fish House Road, Half Mile Sough of the Wittpenn Bridge, Kearny, NJ. Employees were exposed to struck-by hazards from the steel plates in the up to 7'2" deep excavation as according to manufacturer's tabulated data, a minimum of 2 hydraulic cylinders should be in each vertical plane at such a depth whereas the employer only provided for 1 hydraulic cylinder between the plates. Violation observed on or about 06/28/2019.
