29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On the Hebi Haichang Special Equipment Co. LTD machine, EM00124, the guarding openings were too large, allowing access to moving components, thereby exposing workers to amputation hazards. (b) On the Molex brand machine, EM200035, the guarding openings were too large, allowing access to moving components, thereby exposing workers to amputation hazards. (c) On the Hebi Haichung Special Equipment Co. LTD machine, EM200126, the guarding openings were too large, allowing access to moving components, thereby exposing workers to amputation hazards. (d) On the Taber Company Press machine, EM20027, the guarding opening were too large, allowing access to moving components, thereby exposing workers to amputation hazards. Precision Cable Assemblies, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1109776, citation number 1, item number 2b and was affirmed as a final order on May 26, 2016, with respect to a workplace location at 16830 Pheasant Dr., Brookfield, WI 53005.
