29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) Pallet Mill Machinery; On or about November 14, 2017 and at times prior thereto the employer exposed employees to potential hazardous energies in that the employer had not provided authorized employees locks for isolating and securing machine energy sources including but not limited to the Baker 5 Head Saw.
