OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) On or about September 11, 2017 and times prior; Mechanic Shop Area: Knockout holes and openings were not closed exposing employees to arc flash, electrical shock, fire and explosive hazards on the following equipment: i. Bench Grinder Station: Open knockouts on suspended electrical receptacle box 110volt, ii. Shop Main Electrical Panel Box: Bottom of box has corroded openings/holes and is not sealed or contained, iii. Electrical Panel Box - 200 amp (Left of "Shop Main" panel box): Missing knockout on the bottom of panel box, b) On or about September 11, 2017 and times prior; Tool Room Area (Shipping/Receiving Station): Knockout hole and opening was not closed exposing employees to arc flash, electrical shock, fire and explosive hazards.
